Let me take this opportunity to welcome you to my personalized Wave in-depth case by case consultation services. Since you are on this page, you have decided that you are in need of some individual attention. Perhaps this is just a very difficult case and you are looking for a "one-shot" fix. Perhaps you have been somewhat frustrated by your design attempts and fail as often as you are successful. Perhaps you have now decided to take the plunge into custom gas permeable multifocal lenses and are not sure where to begin. Maybe you have been working with Orthokeratology in some "pre-packaged" form i.e. CRT, BE, VST, DreamLens, etc. but now would like more control over your designs. Help Support Defined

The fees you incur utilizing my consultation services are based on the nature of the case. The fees you will be paying for my services are calculated using a direct proportional fraction of the fees that I charge my patients for these very same types of cases. I have structured my consultation fees this way based on certain considerations. I have been providing this type of care for my patients for over a quarter of a century and I know exactly how much time, effort, and costs are involved for each of these types of cases. If your "retail" fees are similar to mine, this direct proportional fractional approach will allow you the opportunity to not only learn from the case, but also to cover your costs of consultation and the costs of doing business. There may even be a small amount of profit for you but the primary goal of these cases and having the fees so structured, is to provide you the guidance and help without additional out-of-pocket expenses. When hiring me for a case, you have hired me to co-manage that case with you for three months from the time we start. This allows additional help on the case well beyond the initial design of the first pair of lenses. Subsequent fitting issues, physiological problems, clinical pearls and "tricks", handling the over-refractive data should all be addressable within that initial three month period. Additionally, if you have the ability to shoot slit-lamp digital photography or better yet, slit-lamp videography, but at the very least cell phone photos and video, I encourage you to send this information to me for follow-up analysis, as well as possible additional design modifications. This affords this co-management relationship of some of these very difficult cases, the greatest chance of success, but please understand that success cannot be guaranteed due to many factors that include but are not limited to, the remote nature of the clinical support.

 

Directions For Correct Usage

Please read these directions very carefully for the correct usage of my consultation services, to avoid costly mistakes. The process of consultation begins on this page with the Case Selection Table below. In the table, you will see a 3 step selection process proceeding from LEFT TO RIGHT and BOTTOM TO TOP. This is where you will specify the type of case with which you would like my help. The proper way to use this table is to select the highest line item that best represents your particular case.

Begin with the column labelled "Step #1" and choose the highest position on the table of the 3 boxes that best represents your case. Then proceed to "Step #2" and choose the highest position line item next to the box that you chose in Step #1 that continues to best describe your case. All gas permeable designs except for the single line item labelled "Scleral Gas Permeable" will be corneal gas permeable design lenses. Proceed to "Step #3" on the same line that you selected in "Step #2." In this box you will notice an "Add to Cart" button (or two "Add to Cart" buttons if there is a single vision or multifocal option) and perhaps a "drop down menu" selector. If there is a drop down menu selector in your box please click on the arrow to further sub-categorize your case. Choose the best sub-category that represents your case. Once this is selected, you can then press the "Add to Cart" button (if there is a choice of single vision or multifocal, please choose the correct "Add to Cart" button within this box).

You will then be taken to a page that will show you exactly what type of case you have chosen and the associated consultation fees. You can view this page at anytime by pressing the "View Cart" button at the bottom right of the Case Selection Table. If you want help on only one case at this time, you can then proceed to "checkout." If you would like help on multiple cases, you can click the "continue shopping" button and you will be returned to this page. You can now add each type of additional case to your "cart." When you are done with the case selection process, you can proceed to "checkout" which will take you to another page to submit payment.

On this page there will be a summary of charges. Payment will be through Paypal which is safe, secure and easy for you to use. You do NOT need a Paypal account to use this form of payment. Paypal can accept all major credit cards, debit cards, bank transfers, and e-check. There is a button to click if you don't have a Paypal account that will take you to a billing information page that will allow you to pay by credit card. If you would like to pay using a Paypal account, the signup process is very fast and easy.

After payment is made successfully, be patient. I will be setting up a folder within my BOX account for you with your name (BOX is an online cloud service for file storage among other things). Once I set up this folder with your name within my account, I will "share" the folder with you which will cause BOX to send you an email to the address you provided to me upon payment. That email will instruct you how to set up your own (if you don't already have one) FREE BOX account. Once set up, you will see the folder with your name and my name, that I have shared with you within your main folder. Click on our shared folder and you will then see a "read me" text file. Click on the read me text file and a preview will open with additional information regarding the use of my consultation services. After reading this information, you can return to our shared folder upload your case to that folder. Once the file(s) are there, please enter a "comment" with all of the pertinent information to the case. After you click "Add Comment" I will be notified that you've entered that comment automatically by BOX. I will also automatically be notified that your files are there waiting for me. You need not do anything else.

After the funds from your payment have cleared, my turnaround time under most circumstances is within 3 business days but generally only 1 business day is necessary.
